Replace a Lost, Stolen, or Damaged Passport or a Passport that Expired over 5 Years Ago

The procedures below apply in non-emergency situations. If your passport was lost or stolen and you have an emergency need to travel in the next 2-3 weeks, please click here for more information about emergency passports.
Replace a Lost or Stolen Passport
Print the Checklist of Requirements by clicking here and follow the instructions below:
Step 1:
- Fill out electronically and print both the Form DS-11 Application for a U.S. Passport (wizard format) and Form DS-64 Statement Regarding a Lost or Stolen Passport (PDF format). If you experience trouble with the DS-11 Form Wizard, click here for the Form DS-11 (PDF Format). To view PDF files, you need a PDF reader such as Adobe Acrobat. Get a free download from the Adobe Website.
- Check that you have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaims and click submit.
- Click Report a Lost or Stolen Passport.
- Print the DS-11 and DS-64 forms upon completion.
- Please note that some instructions and options on the form are only applicable in the United States.
Step 2:
Schedule an appointment. Use a valid email address and print your confirmation.
Step 3:
Please bring the following documents to your appointment:
- The Checklist of Requirements, signed and dated
- DS-11 and DS-64 forms
- Printout of appointment confirmation
- Two pieces 2" x 2" photos
- Police report (if passport was stolen)*
- Valid photo ID* (e.g. valid passport or two other government-issued photo ID, such as driver license, State ID card, SSS, BIR, voter’s ID, etc). Postal ID is not acceptable.
- Photocopy of missing passport (if available)
- Fee: $135 for 16 years old and older; $105 for applicants age 15 and below. Payment may be in cash (U.S. dollars or Philippine peso equivalent) or credit card.
NOTE: Special requirements, such as the Department of State's Two-Parent Consent Policy, apply for applicants who are under 16 years old. Please refer to the instructions on this website found under "Renew a Passport for a child under 16 years old", specifically the information found on Steps 3 and 4.
*Also bring one photocopy along with the original. Otherwise, you will be charged for the photocopying fee of $1.00 per page at the Embassy.
Step 4:
Once payment has been made and application has been approved, passport will be processed and delivered within 2-3 weeks.
Replace a Damaged Passport
