Replacement Report of Birth Abroad

The approved Consular Report of Birth Abroad of a Citizen of the United States of America (FS-240) is evidence that a person acquired United States citizenship at birth.  This form is a basic citizenship document. 

Effective January 19, 2011, the Department of State introduced a redesigned Consular Report of Birth Abroad (CRBA, or Form FS-240).  The redesigned CRBA will be printed in the United States, using the information provided by overseas posts.  The Department will no longer issue the DS-1350 Certification of Report of Birth Abroad (for replacement, amendments or additional copies). Instead, the Department will simply provide new FS-240s in response to these requests.
