Correct or Change Information in Your Passport (name change or typo error) or Replace Your Limited Passport

Follow the instructions below if your passport was issued less than 1 year ago and you need to change information in your passport (either due to marriage or court order or correct a typo error)  

OR

If you need to replace your limited passport* that has not expired yet.  

*If your limited passport has a special endorsement at the back of your passport, it may need prior approval from the Department of State.  In which case, please follow the instructions indicated on the letter given to you when your limited passport was issued.

Step 1:

Fill out electronically and print the DS-5504 Application for a U.S. Passport wizard form.   If you experience trouble with this Passport Application Wizard, click here for Form DS-5504 (PDF format).  To view PDF files, you need a PDF reader such as Adobe Acrobat.  Get a free download from the Adobe website.

Step 2:

Mail your documents to the embassy at the following address: Citizenship and Passport Unit, American Citizens Services, Consular Section, U.S. Embassy, 1201 Roxas Boulevard, 1000 Manila.  Note: there is no need to book an appointment for this service.  You may also call (02) 879-4747 to have Air21 courier pick-up the documents and deliver them to the Embassy. Please include the following:

  • Completed and signed DS-5504 application form
  • Your current passport
  • Two pieces 2" x 2" photos
  • Original marriage certificate* (must be NSO-issued if married in the Philippines) or original court order documenting the name change*
  • There is no fee for this service.

*Also include one photocopy along with the original. Otherwise, you will be charged for the photocopying fee of $1.00 per page at the Embassy.

Step 3:

Once payment has been made and application has been approved, passport will be processed and delivered within 2-3 weeks. The expired U.S. passport (duly cancelled by the Embassy) will be delivered together with the new U.S. passport.

Follow the instructions below if your passport was issued more than 1 year ago and you need to change information in your passport (either due to marriage or court order or correct a typo error)  

OR

If you need to replace your expired limited passport*.

*If your limited passport has a special endorsement at the back of your passport, it may need prior approval from the Department of State.  In which case, please follow the instructions indicated on the letter given to you when your limited passport was issued.

If you are above 16 years old, click here for the checklist of requirements and follow the instructions below. 

Step 1:

  • Fill out electronically and print the DS-82 Application for a U.S. Passport wizard form. If you experience trouble with the Passport Application Wizard, click here for Form DS-82 (PDF format).
  • Check that you have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaims and click submit.
  • Print out the DS-82 form upon completion.
  • Please note that some instructions are only applicable in the United States.

Step 2:

Step 3 :

Once your documents are received, the Embassy will notify you through e-mail for the instructions on when to come to the Embassy to pay the application fee.  Alternatively, you may send a representative to pay the application fee at the Embassy on your behalf.  Please bring: 

  • $110 in cash (U.S. dollars or Philippine pesos equivalent) or credit card
  • A printout of the email notification sent to you by the Embassy
  • If only a representative will come to the Embassy, the representative must have an authorization letter from the passport owner as well as a valid photo I.D.

Step 4:

Once payment has been made and application has been approved, passport will be processed and delivered within 2-3 weeks. The expired U.S. passport (duly cancelled by the Embassy) will be delivered together with the new U.S. passport.

If you cannot legally document your name change (through court order or marriage) and/or if you cannot legally document your name change and your passport was issued more than 1 year ago:

Print the Checklist of Requirements by clicking here and follow the instructions below:

Step 1:

  • Fill out electronically and print the DS-11 Application for a U.S. Passport wizard form. If you experience trouble with this Passport Application Wizard, click here for Form DS-11 (PDF format).  To view PDF files, you need a PDF reader such as Adobe Acrobat.  Get a free download from the Adobe website.
  • Check that you have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaims and click submit.
  • Click Apply Online.
  • Print the DS-11 form upon completion.

Please note that some instructions are only applicable in the United States.

Step 2 :

Schedule an appointment. Use a valid email address and print your confirmation.

Step 3:

Please bring the following documents with you to your appointment:

  • The Checklist of Requirements, signed and dated 
  • Your current passport
  • At least 3 public records showing your date and place of birth that show exclusive use of your assumed name for at least 5 years*
  • DS-11 form
  • Two pieces 2" x 2" photos
  • Printout of appointment confirmation
  • $135 in cash (U.S. dollars of Philippine pesos equivalent) or credit card for applicants 16 years old and above.

*Also bring one photocopy along with the original.  Otherwise, you will be charged for the photocopying fee of $1.00 per page at the Embassy.

Step 4:

Once application has been approved, passport will be processed and delivered within 2-3 weeks. The expired U.S. passport (duly cancelled by the Embassy) will be delivered together with the new U.S. passport.
